PHP-Blotter

PHP-Blotter is a fast, free (GPL), customizable, online note-taker app. Your notes are referred to as blots, they can be either public or private, styled to look like source code or plaintext, and later can be revised and deleted. User registration is required.

Somewhat modern version of MySQL
Should be web server agnostic, as long as PHP is supported
